The RN has the responsibility to perform early and ongoing discharge assessments addressing clinical, psychosocial, and environmental needs.
Develop individualized discharge plans and collaborate in interdisciplinary rounds. Provide patient and caregiver education using evidence based frameworks and teach-back.
Manage and run UM Committee
Document all discharge planning activities in compliance with CMS and accreditation standards
Direct Screen Colonoscopy program coordination
Required Skills
Graduate of an accredited RN School of Nursing.
- Experience 2+years in acute care experience; preferred experience in case management, GI, wellness programs
- Current RN license in the state.
- BLS
- Current ACLS / PALS preferred or included in education plan
- Ability to make decisions based on nursing judgment.
- Able to prioritize and organize workload to meet needs of patients and unit.
- Strong communication skills, verbal and written.
