Director of Quality, Risk & Infection Prevention Surgical Institute of Reading
- Develops and implements a performance improvement plan that meets regulatory requirements. Reviews the plan annually and modifies plan as needed.
- Assists in the aggregation and analyzing of data and reports findings to appropriate committees.
- Coordinates the Quality/Performance Improvement Committee monthly meetings. Applies recommendations into policies/procedures/practices as appropriate.
- Assists in evaluating new products, techniques and/or methods and provides appropriate feedback for improvement purposes.
- Knowledgeable of facility Performance Improvement Program, and role of department and individual employee.
- Works with facility leadership to ensure all staff are oriented and receive ongoing education regarding the Performance Improvement Plan.
- Aggregate performance improvement findings for presentation to Medical Staff and Professional Activities Committee.
- Participate on Performance Improvement teams to improve processes whenever possible.
- Implements actions, as appropriate, recommended by Performance Improvement teams.
- Knowledgeable of identification and reporting of a Sentinel Events, and other incidents via PAPSRS/internal reporting system.
- Facilitates the root cause analysis process as needed.
- Maintains knowledge of regulatory requirement (State and CMS) and accreditation standards related to Quality Assessment and Performance Improvement.
- Manages required regulatory reporting and improves compliance with those measures.
- Active participant in accreditation surveys.
- Chairs the patient experience committee.
- Performs failure mode effect analysis as required on a high risk, high volume/low volume problem prone process and implements all of the required action items.
- Models and facilitates continuous survey readiness.
Qualifications
- Bachelor’s Degree in Nursing. Master's Degree preferred.
- Five years’ leadership experience in Quality, Risk and Infection Prevention.
- Active RN license in the state of Pennsylvania.
Occupational Exposure: Category III exposure to blood borne pathogens, frequent exposure to chemical hazards
Physical demands/working conditions: Requires prolonged sitting, some bending, stooping and stretching. Requires and manual dexterity sufficient to operate a keyboard, copier, telephone, fax machine and other office equipment. Occasionally requires lifting up to 20 pounds.
