Coordinator, Social Work (4104)

The Social Work Coordinator provides administrative and programmatic support to the Social Work Department. This position helps coordinate the daily activities of clinical social workers, assists with case management tasks, and facilitates communication between the social work team, healthcare providers, patients, and families. The Social Work Coordinator ensures that patient needs are addressed in a timely and efficient manner and supports the department’s efforts to provide compassionate, patient-centered care.

• Strong organizational and time-management abilities.
• Effective written and verbal communication.
• Ability to work collaboratively in a multidisciplinary healthcare environment.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and/or electronic health record systems.
• Ability to handle sensitive information with professionalism and confidentiality.

• One (1) year of experience in a healthcare, social services, or related setting.
• Familiarity with medical terminology and hospital discharge planning processes.
• Experience interacting with patients and families in a supportive role.
