The Admissions Coordinator coordinates admission referrals into the Transitional Care Unit (TCU). Develops and nurtures positive relationships with internal and external referral resources. Generates new referral sources and promotes awareness in the community of the rehabilitation services available at PIH Health. Serves as a role model and provides clinical expertise to the department.

- Knowledge of the scope and types of rehabilitation and skilled nursing services and able to determine the most appropriate level of care and type of services for a post-acute care patient
- Knowledge of third-party payer systems, medical insurance, managed care processes, and Medicare requirements
- Creative, motivated, goal-oriented, and able to complete assignments independently
Required Experience
Required:
- Active California RN License
- Current BLS certification from the American Heart Association
- Must obtain LA City Fire Card within 6 months of employment (PHGSH Only)
- 2 years clinical experience in an acute care or post-acute care facility.
Preferred:
- BSN or currently enrolled in a BSN Program
