am hired by the Library, and the Library discovers during my employment that
any statement or answer on this application is false, misleading, or
incomplete, I may be terminated immediately. If I am hired, this application
becomes part of my official employment record.
I agree to submit to a medical examination
which may include testing for drugs before beginning work with the Library. If
I am employed by the Library, I may be required, when job-related and
consistent with business necessity, to undergo a medical examination. I further
understand I may have to submit to testing for illegal drugs or alcohol,
consistent with Library policy.
Nothing in this application creates a
contract of employment between me and the Library. If I am hired, my
employment and compensation are “at will,” meaning my employment can be
terminated, either by the Library or me, with or without cause and with or
without notice. Only the Director or the Board of Trustees has the authority to
contract with me for any specified period of time. If hired, I agree to abide
by all library rules and regulations, including those contained in the
Worthington Libraries Personnel Policy Manual.
If I become personally indebted to the
Library, I authorize the Library to withhold from my wages such amounts as permitted
by law to satisfy my obligation to the Library.
I agree that any claim or lawsuit relating
to my service with the Library must be filed no more than six (6) months after
the employment action that is the subject of the claim or lawsuit. I waive any
statute of limitations to the contrary.
I give the Library permission to
investigate the information in my employment application, which the Library
deems necessary to determine my qualifications for a job with the Library. I
give the Library permission to contact any former employer, educational
institution, credit or finance bureau or office, any personal or professional
reference, or any other source or individual to gather any information,
personal or otherwise, that such sources may have about my character, general
reputation, credit, education, or employment record, and I give my consent to
any such source to release to the Library whatever information they have about
me. I also unconditionally release all named and unnamed sources from all
liability which might result from furnishing any information about me. I
further give the Library authorization to speak to potential future employers
about my work performance and other employment-related matters if I become
employed by the Library.
