The Nurse Family Partnership® Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors are responsible for maintain the highest standards in clinical nursing practice; adhere to the NFP model, and to the policies, procedures, guidelines and standards of NFP and TVCCA.
· Provide home visits to women and their families eligible for the NFP Program
· Provide home visits to the NFP service area which includes New London, Middlesex, and Windham counties
· Adhere to nursing process and the NFP model of home visitation
· Maintain a minimum case load of 21-25 clients
· Provide and document ante/post-partum skilled nursing visits to NFP mothers
· Support and implement all policies, procedures, guidelines and standards of NFP/TVCCA
· As requested/required, participate in community coalition/activities that support NFP/TVCCA
· Assist with NFP client referral and recruitment activity
***TVCCA will offer a ONE TIME SIGN on BONUS of $5,000 ($2,500 after 3 months and $2,500 after 6 Months of employment) for this position.
Required Skills
· Unrestricted registered nurse license
· Valid driver’s license and insured automobile required
· BSN required
· Minimum 2 years of maternal, infant clinical nursing experience, such as postpartum, lactation consultant, home visiting, or mental/behavioral experience
· Able to work with culturally diverse populations
· Able to move or lift at least 15 pounds
· Proficient with laptops, electronic health records, and Microsoft Office applications
· Demonstrated experience in early childhood and maternal services, knowledge of issues related to pre-natal and post-partum needs
· Knowledge of Nurse Family Partnership and CT Office of Early Childhood (OEC) standards including reporting and funding source requirements preferred
· Strong written and verbal communication required
· Must have strong decision making, organizational, and leadership skills
· Experience leading and managing employees and multiple projects simultaneously with differing deadlines
· Ability to travel to/from home visits throughout the entire NFP service area
· Work a flexible schedule including occasional evening sand weekends
· Bilingual preferre
Required Experience
Complete all required NFP and Maternal, Infant, and Early Childhood Home Visiting (MIECHV) education
· Attend and participate in DANCE (Dyadic Assessment of Naturalistic Caregiver-Child Experiences)
· Implement and document DANCE assessments according to NFP schedule
· Implement and document developmental screening and other assessment screenings according to NFP and MIECHV schedules
· Implement and document Partners in Parenting Education (PIPE) curriculum with mothers and families
· Perform home visits in accordance with NFP Model and NFP Visit-to-Visit Guidelines
· Develop therapeutic relationships; utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment
· Establish and maintain professional boundaries
· Use the nursing process for patient-centered care while implementing NFP into nursing practice
· Assess physical, emotional, social and environmental strengths and risks of women and their families as they relate to the six NFP domains using the Strengths and Risks Framework (STAR)
· Assist women and their families to establish short- and long-term goals
· Formulate nursing diagnosis based on nursing assessment and client goals
· Plan, document, and evaluate the nursing plan of care based on nursing diagnosis and the progress made toward stated goals and NFP outcomes
· Provide and document in the nursing care plan the education, support, and referral resources used to assist women and their families to attain their identified goals
· Evaluate client progress toward stated goals and NFP outcomes; plan home visits in accordance with client goals and NFP outcomes
· Document nursing assessments and care plans on-time and correctly
· Consult and collaborate with other professionals providing services to NFP women and families
· Actively engage in professional development to meet all NFP Nurse Home Visitor and MIECHV competency requirements
· Meet with NFP Nurse Supervisor weekly for clinical supervision
· Utilize reflective practice in supervisory sessions
· Attend and participate in bi-weekly case conferences and bi-weekly team meetings
· Schedule and implement joint home visits with NFP Nurse Supervisor
· Provide data to the support staff for timely and accurate data input to the NFP performance management system
· Participate in review and analysis of NFP reports identifying achievements and deficiencies
· Participate in quality improvement efforts
· Maintain a professional nursing image
· Support and participate in activities for NFP mothers and families
· Understand, support, and coach others in the NFP vision, mission, and model
· Ability to use a collaborative approach in community-based work. Strong commitment to diversity, equity and inclusion. Ability to work independently to meet deadlines and work well with multiple priorities
· Assist to create a positive work environment that promotes productivity, mentoring, teamwork, and cooperation
· Represent the NFP program in the internally and externally at relevant meetings and events
· Establish good working relationships and collaborative arrangements with community providers, groups, collaboratives, funders, politicians, and other organizations to help achieve the program's goals
· Assist parents in making appointments with health care providers, social services agencies, and other community referrals; serve as a liaison between families and referrals as needed
· Utilize professional interpreters when interacting with families with limited English proficiency
· Respond to client needs through flexible scheduling of home visits
· Care for diverse populations with cultural humility
· Adhere to, promote, and enforce TVCCA policies, procedures, mission, and philosophy
· Attend statewide and other meetings as necessary; comply with agency meeting schedules and all contractually required meetings
· Maintain confidentiality at all times
· Serve as an advocate for the clients of TVCCA
· Perform additional tasks to meet the needs of the program and agenc
