MAJOR FUNCTION: The Case Management position is a grant-funded position by the Florida Department of Correction (DOC), Jefferson Correctional Facility (Monticello, FL). Case managers provide support services to adult offenders through ongoing assessment, monitoring, coordination of services, referrals, and treatment planning. Case managers provide planning and evaluation for coordination of treatment services for each client to successfully re-enter society. Primary duties will include:
- Serve as a point of contact for clients and responsible for studying, analyzing, assisting, motivating, and coordinating various services that pertain to offenders reentering society. Generate an individualized plan to assist with successful re-entry into society using the Five-Key Model for Reentry model. Network with health care practitioners, law enforcement agencies, and Re-Entry colleagues to gain additional insight of a client’s progress.
- Responsible for studying, analyzing, assisting, motivating, and coordinating various services that pertain to offenders experiencing mental health or substance use disorders and high-risk individuals convicted of serious crimes. Closely monitor on-going cases and documents all case management activities in the system designated to address concern.
MINIMUM TRAINING/EXPERIENCE: Requires a bachelor's degree with a minimum of 15 credit hours in a social services discipline such as social work, psychology, counseling, human services, or a related field and one year of related experience. Preference will be giving for case management experience. Experience in Case Management regarding the needs of adult offenders re-entering society successfully is highly desired. Experience working with mental health clients is a plus.
Candidate must be able to work in non-traditional educational – a state correctional institution. Candidate must be able to pass a DOC background check, which includes fingerprinting. Additionally, the candidate must successfully pass a Tuberculosis (TB) screening. Prefer former teaching experience and a current State of Florida Teacher’s Certificate from a Florida School District, or a Florida Department of Corrections (DOC) Vocational Certificate, and attain or be able to attain a NCCER Instructor Certification and OSHA 10 1926 Card.

Excellent written/oral communication skills (writing /editing, phone follow-up) with the use of proper written and verbal English grammar, vocabulary, and spelling. Ability to speak clearly and distinctly. Ability to navigate the college ERP systems (Workday). Ability to handle and protect confidential data with integrity. Ability to interact with other project staff and DOC staff appropriately. Ability to utilize problem-solving techniques. Ability to keep accurate & thorough records. Ability to access transportation. Ability to organize large amounts of information. Effective time management skills. Ability to Demonstrates energy, organization, and professional demeanor. Ability to use basic office equipment including, but not limited to: Desktop Computer, Printer, Telephone, Photocopier, Fax Machine, Scanner, and Calculator. Organization of workspace/office. Ability to file, organize, and retrieve of computerized and paper records. Ability to plan and execute routine office duties. Ability to manage confidential information and tasks. Ability to use and adapt technologies, including online databases. Ability to follow written and oral instructions. Ability to take initiative with assignments to help improve the quality of office procedures. Ability to work independently with limited supervision. Ability to perform routine office duties including filing, copying, preparing mailings, and departmental requests as needed. Ability to coordinate and manage multiple tasks concurrently. Ability to interact effectively with multiple levels of the public. Ability to function under pressure, meets deadlines, handle multiple requests, and provide detailed follow-through on various tasks. Ability to solve problems and make procedures more effective. Ability to exercise good judgment, discretion, and confidentiality. Ability to work in a diverse, multicultural environment with teams of staff across multiple organizations.
