Great opportunity exists for a Mental Health Caseworker in our Intensive Community Support-Respite program! This program is available to adults referred by the Department of Mental Health (DMH) who have lost their housing, are experiencing a need for increased support, or are transitioning back into the community from a hospital setting. The open, six (6) bed residential unit provides clients with stability as they connect with the resources and services needed to achieve independent living. The Respite Caseworker will be part of a dedicated, multi-disciplinary team of professionals which includes Nurses, Mental Health Workers, Peer Specialists, and Master’s level Clinicians.
Responsibilities include: providing “warp around services” to clients who require intensive short-term care coordination to allow them to mainstream community care. Responsible for facilitating and participating in individual treatment planning, recreational, therapeutic, and coping activities; and encourages consumer health through proper diet and daily living. Completes documentation and date per shift as assigned. Attends and completes all required training including Medication Administration Program (MAP).

Provides active efforts to engage DMH referred clients.

Provides case coordination e.g., program specific treatment planning; coordination of care between behavioral health and medical providers.

Performs Individual Care Plans as directed by the Clinical Care Coordinator, Case Manager or the ICM.
Schedule: Sunday – Thursday 9am – 5pm (40 hours)
Hourly Range: $20.40 - $23.25 (Based on experience) +$1,000 Sign-on Bonus!

Required Skills
- Previous direct care experience with a psychiatric population
- Knowledge of funding sources, housing, and ability to coordinate community based services strongly preferred
Required Experience

High School Diploma required. Bachelor's degree in Psychology or related field preferred.

Valid Driver's License and a good driving record required.

Access to personal vehicle needed for outreach and client transportation required.

MAP certification within six (6) months of employment date required.

CPR/FIRST AID within three (3) months of employment date required.
