Intensive Care Coordinator - Bachelor's Level

 

 

Exciting opportunity to join our innovative and growing children's mental health program as a Bachelor Level Intensive Care Coordinator!

Responsibilities:
 
  • Provide comprehensive care coordination to children and families.
  • Perform initial clinical assessments to identify family and youth's need and goals.
  • Develop, implement and monitor individualized care plans based on family needs and goals.
  • Facilitate care plan team meetings.
  • Act as a liaison with collateral agencies and programs
  • Advocate for families and educate them on self-advocacy
  • Identify and assist youth and families in accessing necessary services
  • Ensure proper documentation, and safety planning
  • Participate in staff meetings, supervision, and training

Rate: $22.28/hourly
 
 
Schedule: Full Time, 40 hours

Required Skills

  • Excellent communication and organizational skills
  • Basic computer fluency, including Microsoft Office
  • Valid driver’s license and reliable vehicle 
  • Bilingual in Spanish or Portuguese highly preferred

Required Experience

  • Bachelor's degree in a mental health-related or human services field (such as social work, counseling, child development, psychology, human services) from an accredited college or university
  • Minimum of one-year experience working with children with serious emotional disturbance
  • Community-based experience strongly preferred
