Position Summary: We have an exciting opportunity to join our team as a Clinical Care Coordinator Per Diem - LOH. In this role, the successful candidate provides clinical leadership and coordination for patient care consistent with the philosophy and goals of the institution, the Department of Nursing, and Care Management Department. Responsible for developing/managing the patient care transition process: for coordinating, facilitating and assisting patients throughout the episode of care. Serves as clinical resource with expertise in musculoskeletal patient care management and serves as liaison throughout HJD regarding services for this patient population. Oversees the process for clinical pathway development, staff training, and data collection and reporting. Serves as a positive role model as a nursing leader. Evaluates and supervises staff in the RN Case Management role. Demonstrates ability to develop, guide, motivate, nurture and coach others. Actively participates in departmental meetings, committees and quality initiatives. Possess strong interpersonal communication skills, customer service orientation and creative problem solving. Actively participates in related professional organizations that provide networking and benchmarking opportunities. Exhibits physical stamina, manual dexterity, visual and aural acuity to perform responsibilities.
Job Responsibilities:
- Provides clinical oversight and direction of patient care issues in support of unit-based operations.
- Participates and fosters staff involvement in evidence-based practice and other nursing research efforts.
- Participates and/or facilitates research protocols applicable to unit, ensures knowledge of staff, protects rights of patients, and participates in assessment/evaluation of outcomes.
- Fosters a positive customer service environment that promotes communication and resolution of issues for patients, families, physician and staff.
- Provides organizational direction for PI/UR efforts.
- Active member of the UR/Care Management Committee.
- Evaluates data including LOS, cost/reimbursement, quality indicators.
- Ensures data collection/outcomes management.
- Implements performance improvement action plans, collects data and identifies additional opportunities for improvement.
- Avoidable days and readmission tracking/reporting.
- Monitoring to prevent variance to LOS target.
- Prospective DRG LOS assignment.
- Oversight/application of Interqual Intensity of Service/Severity of Illness Criteria and other evidence based criteria sets.
- Collaborates with management to support department quality and performance improvement activities consistent with the service and departmental plan.
- Ensures appropriate care delivered for patient age and/or developmental specific needs.
- Serves as support and resource for the education of health care providers as opportunities arise.
- Initiates professional development plan for self to improve and sustain clinical and leadership competencies.
- Educates healthcare team regarding patient plan of care, discharge process and target discharge dates.
- Collaborates with others to develop a learning atmosphere that supports education and development of department staff.
- Collaborates with others in identifying, addressing, and utilizing available resources to meet the learning needs of professional and ancillary staff members.
- Functions as a respectful member of the health care team.
- Participates in interdisciplinary rounds.
- Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities.
- Demonstrates a spirit of collaboration and effective cooperation with other departments within the hospital.
- Monitors and implements plans as needed to ensure regulatory compliance pertinent to the department.
- Promotes excellent customer service for patients, physicians, and others by ensuring expectations are met or exceeded.
- Reviews requests for time off and notifies staff regarding approval or denial in a timely fashion and in accordance with departmental guidelines.
- Initiates disciplinary and corrective action processes as appropriate.
- Participates in recruitment, orientation and professional development of direct reports.
- Conducts timely performance appraisal for direct reports.
- Assists with interviewing and selecting staff for the department.
- Identifies, initiates investigation and proactively responds immediately to potential and/or actual risk management issues.
- Facilitates an environment conducive to creativity in practice, collaboration with all members of the health team, and the fostering of collegial relationships.
- Facilitates communication among attending physicians, other team members, external providers and payers.
- Develops and manages the processes related to pre-admission and post-discharge care transitions; establishes relationships/clinical pathways with providers/agencies to optimize care for specific patient populations.
- Supports the mission, philosophy, standards, goals and objectives of the institution, department and service line.
- Escalates long stay/high cost cases to admin/physician advisor/medical director/service chief.
- Clinical review/discussion with attendings/medical team to advance plan of care/discharge.
- Directs clinically complex discharge plans of care: complex wound care, high dollar meds and/or intensive resource utilization.
- Leads and facilitates patient/family/ healthcare team meetings/care conferences.
- Oversight of patient/family assessment related to clinical plan of care and transition management.
- Facilitates appropriate resource utilization (e.g., MRI, radiology procs, testing, MD consults, Abx usage, etc.)
- Participates in the development, promulgation, and implementation of process standards for the delivery of patient care relevant to scope of service.
- Development and oversight/management of clinical pathways.
- Acts as a professional role model to all levels of staff by incorporating expertise, critical thinking and related experience in care of patients and families.
- Assessment and navigation/transition management services for patients throughout an episode of care.
- Serves as a role model for staff in considering the needs and behaviors of specific patients in a culturally competent manner.
- Assists in facilitating a collaborative interdisciplinary approach to achieve optimal patient care outcomes.
- Demonstrates awareness and recognition of the clinical plan of care along the continuum.
Minimum Qualifications:
To qualify you must have a Licensure: Current Registered Professional Nurse in the State of New York;
Education/Certification: Bachelor of Science Degree in Nursing is required, Certification preferred in relevant areas of practice
Experience: Three to five years clinical experience, acute medical-surgical preferred, or in the care of the population to be care managed. Two or more years in utilization management, discharge planning/case management preferred..
Required Licenses: Registered Nurse License-NYS
Preferred Qualifications: Masters of Science in Nursing or relevant health related field.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $72.00 - $77.05 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
