Position Summary:
We have an exciting opportunity to join our team as a Program Manager of Pharmacotherapy - Brooklyn.

In this role, the successful candidate Responsible for creating and managing a specialized pharmacotherapy clinical program within an oncology team. Work under minimal supervision to troubleshoot pharmacotherapy challenges and manage and supervise all pharmacotherapy needs in an oncology pharmacy clinical program. Responsible for creating, implementing, and managing a clinical pharmacotherapy program with particular focus on participation in a collaborative drug therapy management (CDTM) agreement.

Job Responsibilities:

    1. Manage a clinical pharmacotherapy program within an oncology team via participation in multidisciplinary rounds and/or patient clinic visits, provision of pharmacotherapeutic/pharmacokinetic/pharmacoeconomic consultations, and facilitation of transitions of care as needed. Offer patient and caregiver medication education counseling. Participate in educational initiatives in both the inpatient and outpatient settings.
    2. Develop, implement, and manage a CDTM agreement with physician providers in an assigned hematology/oncology patient care setting in accordance with state and institutional guidelines. 
    3. Evaluate and report measures of patient encounter productivity and identify methods to enhance efficiency of such encounters.
    4. Maintain proficiency in USP Chapter <797> compounding procedures for emergent clinic-based compounding
    5. Actively participate in tumor boards for select hematology/oncology clinical practice areas
    6. Provide oversight for quality assurance of clinical pharmacy programs to improve quality of care, with a particular focus on creation, review, and management of oncology Epic treatment plans and treatment pathways.
    7. Participate in and manage patients within an ambulatory Antimicrobial Stewardship program.
    8. Contribute to professional development of staff, including staff and clinical pharmacists 
    9. Create and manage practice guidelines to enhance pharmaceutical care and medication safety for hematology/oncology patients.
    10. Clinical intervention, adverse drug reaction, and medication error documentation and reporting.
    11. Enterprise committee involvement including Pharmacy & Therapeutics, Medication Utilization Evaluation, Cancer Center Clinical Services, and select hematology/oncology medical committees. 
    12. Responsible for pharmacy resident precepting; pharmacy student mentoring; and scholarly activity.
    13. Identifies and provides expertise for Medication Use Evaluations and monograph authoring for P&T requests. 
    14. Maintains professional competency and certification.
    15. Support high-risk oncology patients through rotating weekend on-call clinical coverage and surveillance of patient charts across PCC sites.

Minimum Qualifications:

  • To qualify you must have a Doctor of Pharmacy degree from an ACPE accredited school of pharmacy
  • New York State Pharmacist Licensure with current NYS registration
  • PGY1 pharmacy residency and PGY2 pharmacy residency training
  • Board certification in Oncology pharmacy (BCOP) within 12 months of hire. 
  • Collaborative Drug Therapy Management Certification within 12 months of hire. 
  • Self-motivation, good communication, organizational, and interpersonal skills.. 

    Required Licenses: Brd Crtfd Oncology Pharmacists, C-Collaborative Drug Thrpy Mgt, Pharmacist License-NYS

Preferred Qualifications:
2 or more years of post-residency oncology clinical pharmacy practice experience after PGY2 completion. Experience working in an antimicrobial stewardship program.

View Know Your Rights: Workplace discrimination is illegal. NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $121,792.21 - $190,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
