Position Summary:
We have an exciting opportunity to join our team as a Supervisor, Social Work - Discharge Planning, Case Management.
Under the supervision of the Director of Social Work, supervises and maintains social work programs for designated sites, clinical service lines and/or departments, patient care units, grant funded special services, or combination thereof. Interfaces with other hospital staff to ensure coordination, communication, and monitoring for the social work staff assigned to service lines, departments, and programs. This is a managerial position that includes direct supervision of professional and non-professional staff.
Job Responsibilities:
- Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of duties
- Institutes corrective actions in consultation with Human Resources/Labor Relations as needed.
- Maintains own clinical competencies and may directly assist staff in psychosocial care and continuing care planning for complex cases as needed
- Is competent to supervise the psychosocial care of patients based upon age groups treated
- Monitors chart recording, statistics and other systems used to measure performance levels
- Partners with Care Managers and Care Management Administration to ensure coordinated efforts in the management of safe and timely discharge planning
- Coordinates staff development needs, discharge planning services, data collection and research, group work and other developmental activities.
- Coordinates graduate social work student activities within the unit and department
- Interviews, selects and recommends hiring of social workers and secretarial staff in assigned areas
- Maintains and develops social work programs within assigned service areas
- Participates in teaching psychosocial issues related to health and illness, community resources, and social work services
- Establishes criteria and case finding mechanisms for ensuring that all patients requiring social work services are identified and treated within timeline established by department policies and procedures
- Insures service to individuals, families and groups through productive use of social work staff time, skills, and appropriate level of competence. Monitors productivity and quality.
- Provide individual and group supervision of both 1199 and non-union staff including social work and clerical staff on a regular basis.
Minimum Qualifications:
To qualify you must have a Masters Degree in Social Work (MSW), or a higher degree from a program accredited by the Council on Social Work Education (CSWE) and recognized by the New York State Education Department as qualifying for licensure, is required.
Licensure as a Licensed Clinical Social Worker (LCSW) in New York State is also required. In certain cases, a limited permit issued by the New York State Education Department may be considered, depending on departmental needs.
A minimum of five years of progressive clinical experience in social work is required, with preference for those who have worked in healthcare, hospital, or home health settings.
At least one year of experience in a supervisory, administrative, or social work teaching role is also necessary.
Knowledge of psychosocial assessments, discharge planning, community resource linkage, and behavioral health interventions is essential.
The role requires excellent verbal and written communication, interpersonal, leadership, and problem-solving skills.
Strong organizational and team-building abilities are critical, along with proficiency in clinical documentation and electronic health record systems.
Familiarity with regulatory requirements and standards in acute care, home health, or oncology settings is expected, as is the ability to manage and support a multidisciplinary team in delivering patient-centered care..

The salary range for the role is $79,698.00 - $128,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
