Position Summary:
We have an exciting opportunity to join our team as a Patient Coordinator - Infusion Department- Full Time, Days - Infusion Department - Monday - Friday 10:00AM-6:00PM, Rotating Saturday.
In this role, the successful candidate As a Patient Coordinator, you will be responsible for ensuring that appropriate patient treatment plans are followed. Coordinates follow-up care plan needs for patients by scheduling appointments or enrolling members in programs, assesses member compliance with medical treatment plans via telephone or through on-site visits.
Job Responsibilities:
- Under the supervision of the Administrative Director, the Patient Coordinator will create a customer friendly environment utilizing exemplary customer service skills.
- The coordinator will greet patients upon entry to the unit, obtain demographic information from patients and family, pre-register patients scheduled for the unit, support the operational flow of the infusion unit and maintain a well-organized and friendly environment.
- Interviews patients to complete documents, case histories, and forms such as intake and insurance forms. Compiles and records medical charts, reports, and correspondence using a computer and various software programs. Transmits correspondence and medical records by mail, email, or fax.
- General office management including but not limited to schedules and confirms patient diagnostic appointments, surgeries, and medical consultations. Appropriately communicates with PAT and patient access for pending procedures. Answers telephones and direct calls to appropriate staff. Receives and routes messages and documents such as laboratory results to appropriate staff. Maintains medical records, technical library, and correspondence files. Performs various clerical and administrative functions such as ordering and maintaining an inventory of supplies.
- Retrieve and enter patient lab results and medication profiles, including follow up on any additional required or outstanding documentation
- Completes patient charts upon discharge along with documenting patient care-related activities and maintaining patient records, manages all paperwork or supportive documentation required for performing effective billing and forwards to billing department. Communicates effectively to patients regarding procedural instructions. Communicates effectively with referring physician offices and acquires all required documentation needed to schedule procedure
- Assists with all other duties as required
Minimum Qualifications:
To qualify you must have a Experience with word processing and computer databases required.
Strong oral and written, multitasking, detail oriented, organizational, customer service and interpersonal skills.
Education and Experience:
High school diploma or GED required.
One year of office experience required; two years preferred.
Preferred Qualifications:
1-2 years scheduling experience in a medical office or a healthcare setting.

The salary range for the role is $38,712.24 - $51,718.88 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
