Position Summary:
We have an exciting opportunity to join our team as a CTC Pediatric Flow Coordinator.

In this role, the successful candidate Coordinates all components of pediatric patient flow activity, including facilitation of all placements, transfers, direct and elective admissions into and within NYU Langone Health. Facilitates and prioritizes transfers in collaboration with Pediatric and Neonatal Transport Nurses, Pediatric CTC Medical Directors, accepting MDs, and members of the interdisciplinary team. Ensures patients clinical needs align with the health systems bed availability. Acts as a clinical resource in decision-making for safe and reliable patient placement. This position also works closely with the leadership team to identify, trend and report barriers to patient throughput.

Job Responsibilities:

  • Perform CTC functions in a responsive manner responding to all callers with the aid of the Electronic Medical Record (EMR), approved clinical protocols, established patient care protocols and healthcare information.
  • Serve as CTC Pediatric team lead to ensure strong communication between all team members.
  • Assist in data collection, monitoring, evaluation, and analyzing of defined quality and throughput measures for the purpose of developing and implementing protocols, policies and procedures.
  • Stay up to date with all departmental procedures, policies and communications to ensure adherence of appropriate processes.
  • Proactively anticipate patient placement requests.
  • Ensure timely transfer and appropriate patient placement in the EMR.
  • Lead twice-daily bed huddles and necessary surge huddles; effectively communicate resources and issues to leadership appropriately.
  • Communicate and prioritize patient placements, including formulating an accurate picture of the patient and his/her condition and informing of incoming admissions to accepting attending physicians, nurses or other clinicians.
  • Facilitate patient flow through the continuum of care by leveraging systematic processing of patient acuity, proactive rounding and expert level clinical decision-making.
  • Demonstrate strong clinical assessment skills & clinical judgement to appropriately assess patient acuity and needs, assume principal responsibility for the coordination of patient acceptance, admission and transfer, and necessary communication between the referring physician, attending physician and clinical staff. 
  • Provide outstanding customer service/patient experience by answering phone calls and requests in a friendly and positive manner, and resolve all inquiries in a timely fashion. 
  • Monitor & document all aspects of the patient transfer process. Obtain and document/uploads all available patient medical information from referring facility (e.g. face sheet, lab results, H&P, current medications, etc.) in the EMR. 
  • Facilitate on-line physician conference calls with Referring & Accepting Attending Physicians (and other key stakeholders, as necessary) based on service and level of care requested for all incoming transfers. 
  • Appropriately prioritize all incoming patient transfer requests in consultation w/ accepting/referring physicians, Pediatric CTC Medical Directors, and in accordance with department policies & procedures.
  • Initiate the financial clearance process via Revenue Cycle Operations and/or Patient Access, and coordinate with referring MD offices or the referring Case Managers to obtain insurance authorization for patient transfer. 
  • Serve as a primary liaison throughout the patient transfer and direct admission process.
  • Submit patient transfer request report w/ all pertinent clinical information to Pediatric CTC Medical Directors (and to other key stakeholders as indicated) for review and transfer approval. 
  • Confirm patient acceptance with accepting/admitting attending physician according to acceptance guidelines for each respective receiving location. 
  • Collect & document patient clinical updates & current vital signs from the referring facility throughout transfer process at regular intervals in accordance with department policy. 
  • Facilitate nurse to nurse patient handoff/report via conference call for all approved transfers. 
  • Arrange or coordinate EMS transportation in partnership w/ outside organizations, and communicate all pertinent transport updates (including estimated time of arrival) to the accepting/admitting attending physician and other key stakeholders as indicated. 
  • Arrange clinical accompaniment by a physician, RN, RT or other clinician during EMS transport as indicated for critically ill patients. 
  • Monitor EMR to ensure safe patient arrival at destination. 
  •  Other duties as assigned.

 

Minimum Qualifications:
To qualify you must have a Current unrestricted New York State RN license.
BCLS BSN
Three years of related clinical experience in a pediatric acute care setting (Emergency Department or Critical Care Preferred) or five years of pediatric Medical/surgical/OR/PACU experience.
Proficient in EMTALA.
Strong problem-solving, multi-tasking, analytical/critical thinking and conflict resolution skills.
Ability to establish and maintain collaborative and effective working relationships.
Ability to communicate effectively in oral, written and electronic formats (i.e. email, electronic medical record, etc.)
Excellent telephone manner and use of professional vocabulary.
Ability to function calmly in a stressful situations and fast-paced environment.
Positive, enthusiastic, customer-service oriented personality.
Proficiency in Microsoft Office suite (Outlook, Word, Excel, etc.).

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $118,613.99 - $126,750.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
