Jennie Stuart Health is seeking a dedicated Case Management Social Worker to join our healthcare team on a PRN basis. This role is pivotal in providing comprehensive social services and discharge planning in accordance with professional standards and hospital policies. The Case Management Social Worker serves as a critical link between physicians, patients/families, and the care team throughout the healthcare delivery process.

As a Case Management Social Worker at Jennie Stuart Health, you will be responsible for identifying and implementing strategies to enhance efficiency, quality, patient safety, customer satisfaction, and revenue generation within the case management standards of practice and your licensure scope. 

In this role, you will conduct thorough initial and follow-up assessments regarding patients' histories, current statuses, acute care needs, and post-acute care requirements. Your documentation will reflect psychosocial and medical backgrounds of patients, including high-risk screenings and proposed interventions. You will utilize sound judgment and clinical knowledge to determine appropriate interventions addressing patients' needs and barriers to progression through the healthcare system.

A significant aspect of this position involves proactive discharge planning, which includes coordinating patient/family education and developing/implementing safety and quality-driven post-acute care plans. You will actively collaborate with interdisciplinary team members and guide interventions regarding ethical/legal issues, abuse/neglect cases, adoptions, and family conferences. As a patient advocate, you will effectively represent patients' and families' desires in planning and negotiations.

The ideal candidate will demonstrate knowledge about coverage parameters for acute care stays and potential discharge plans, keeping patients/families and physicians informed of findings. You will understand utilization management necessity for all patients regardless of third-party payor status and take the lead in cases with challenging financial barriers.

Your responsibilities will include participating in interdisciplinary care conferences, consulting with internal and external healthcare providers, making referrals for continuity of care, and regularly communicating with physicians regarding barriers to discharge, delays, medical plans, and other key case management factors.

As a Case Management Social Worker, you will serve as a resource for issues related to case management/social work and mentor others. You will communicate with the care team to help patients move through the Jennie Stuart Medical Center system efficiently and work with the interdisciplinary team to identify opportunities for process improvement and enhanced quality outcomes.

This position requires excellent customer service skills and effective hand-off communication. Within your scope of practice, you will assist the organization and patients/families with issues such as patient flow challenges, advance directives/decisional capacity, psychiatric patient referrals, and end-of-life issues. You will establish working relationships with patients and families, allocate increased time for difficult or complex cases, and educate them regarding available resources for physical and psychosocial needs.

Required Experience

  • Minimum of one year clinical experience
  • Experience in acute-care setting (preferred)
  • Experience with discharge planning
  • Background in continuous performance improvement
  • LSW in the state of KY (preferred)
  • CCM certification (preferred but not required)
  • Experience with interdisciplinary care conferences
  • Background in patient/family education
  • Experience with healthcare resource coordination
  • Familiarity with advance directives and end-of-life issues
  • Experience with psychiatric patient referrals
