The Clinical Oncology Pharmacist is embedded in the Medical Oncology Clinic and/or Infusion Center to serve as the oncology medication expert.  This position reports directly to the Director of Medical Oncology.

The Clinical Oncology Pharmacist will assist in the optimization of medication management and participate in clinical decision making.  The responsibilities will include but are not limited to direct patient care, oversight of specialty med program, and chemotherapy regimen order completion.

ESSENTIAL FUNCTIONS

Customer Services:

  • Welcoming patients and visitors.
  • Answering and referring inquiries within scope of practice.
  • Telephone: Answers telephone, processes call, and document appropriately within medical records. Returns all messages in a timely manner.
  • Maintains communication between medical staff, administrative staff, and/or patient/families.

Clinical Services:

  • Round with inpatient multidisciplinary teams as appropriate and participate in multidisciplinary clinics.
  • Provide clinical checks on all chemotherapy orders
    • Clinical review includes but not limited to reviewing height, weight, dosing, pathology, staging/scans, supportive care regimens, and appropriateness of therapy in addition to the standard chemotherapy order review items listed above.
    • Verify each chemotherapy order after performing a clinical check reviewing the components listed above.
  • Monitor new and ongoing therapies by reviewing daily labs, tests, and procedure results.
  • Provide pharmacokinetic drug monitoring for medications with a narrow therapeutic index (ex. Methotrexate, aminoglycosides, vancomycin, etc.)
  • Review and update medication lists to monitor for drug-drug, drug-disease, and drug-food interactions.
  • Assist in patient adherence monitoring to both intravenous and oral therapies.
  • Monitor and document side effects/adverse drug reactions.
  • Counsel patients and/or families on chemotherapy regimens, antiemetics, growth factors and other supportive care medications.
  • Maintain appropriate communication with interdisciplinary teams.
  • Comply with all applicable standards of regulatory agencies.
  • Participate in the management of chemotherapy extravasation.
  • Write and assess need for total parenteral nutrition
  • Additional monitoring parameters are included for specific oncology-related medication issues including;
    • Management of immunosuppressant therapy adverse drug events
    • Oncology Supportive Care
      • Antiemetic management
      • White blood cell factor support management
      • Anti-infective prophylaxis recommendations for at risk populations
      • Venous thromboembolism prophylaxis
  • Assist with development and provide oversight for oncology oral medication program within the Medical Oncology Clinic.
  • Non direct patient care related to activities performed by the Clinical Oncology Pharmacist include but are not limited to:
    • Utilization, ongoing development and revision of computerized physician order entry sets and written order sets within specified EMR.
    • Formulary review of hematology and oncology drugs and management of oncology and non-oncology medications in the outpatient setting.
    • Development and maintenance of policies and membership on committees to ensure optimal medication management in the oncology setting.
    • Maintenance of adequate education materials for patients and caregivers
    • Participation in clinical research and quality improvement activities to benefit patient care and oncology medication management.

 

Environmental Services:

  • Ensures all equipment in exams and surgical rooms are clean and properly set up.
  • Contributes to the overall cleanliness and appearance of personal work space and department.
  • Monitors clinical supply levels in exam rooms and stock room and requests when needed.
  • Maintains all clinical equipment.  Evaluates all equipment for damage or maintenance needs and reports when needed.

Required Experience

Minimum Education: Must be a graduate from an approved school of Pharmacy.  Successful completion of a PGY1 and PGY2 Pharmacy Practice Residency program (preferably a PGY2 Hematology/Oncology Pharmacy Residency)

Minimum Work Experience: One year working experience in a clinical setting as a Pharmacist. Prefer 3+ years of experience working with an Oncology Program.

Required Licenses: Pharmacist.  Must be licensed by the State of Kentucky as a Pharmacist. Must be able to demonstrate the ability to provide care according to licensure requirements.  Board Certification as an Oncology Pharmacists preferred.
