Comagine@ Health is looking for a Southern Nevada Pathways Community Hub Manager to oversee the Southern Nevada Pathways Community Hub (Hub), operated by Comagine Health. This position will manage and direct the day to day work of Hub staff and subcontracted Care Coordination Agencies (CCAs), ensure the Hub is operating in adherence to requirements outlined in the Operations Manual as well as in compliance with the Pathways Community Hub Institute (PCHI) certification requirements, and ensure the Hub is executing on contracts and grants that are funding its implementation with quality, on time and within budget. This role will lead the Southern Nevada Pathways Hub team and portfolio of contracts and grants focused on increasing the capacity and sustainability of the Hub to serve those most in need in Clark County, Nevada. While this is a remote work position, the person in this role must reside in Clark County, Nevada and be able to participate in and lead in-person community and team meetings.
What is the Pathways Community HUB Institute® Model?
The Southern Nevada Pathways Community Hub is currently implementing the PCHI model and seeking certification from the Pathways Community HUB Institute®. The PCHI Model is an evidence-based model for coordinating a network of CCAs who employ community health workers (CHWs) to identify and address risk factors, including health related social needs, for at-risk members in the community. The Hub, as the quality center of the network, assures collaboration in approach to identifying and addressing risk, streamlining referrals to reduce service duplication and fragmentation, and increasing the effectiveness of systems of care. Community-based CHWs conduct outreach to identify individuals in need and maintain their relationship with the clients until they graduate from the Hub — regardless of insurance status or health care provider.
The Pathways Community HUB Institute Model has been recognized by the Agency for Healthcare Research and Quality (AHRQ), Centers for Medicare and Medicaid Services (CMS), Health Resources and Services Administration (HRSA) and the Centers for Disease Control (CDC) as a method to integrate services, improve outcomes, reduce duplication, and lower costs. The PCHI Model meaningfully engages hard to reach members to close gaps in care while streamlining contracting, standardizing data, limiting risk, leveraging resources, and producing a return on investment with measurable health improvement.
