Purpose
This position conducts in-home assessments for developmentally disabled participants electing to receive services through the New Mexico Mi Via Waiver as well as supervisory duties.
· Provides operational supervision of HCBS assessors staff who conduct in-home assessments.
· HCBS supervisor will guide, train administrative and field assessor to include assist with contract management for external assessor services.
This important work includes administering face-to-face assessments, coordinating meeting schedules with participants (and their designated agents), and documenting assessment results into the care management system within required due dates.
Education
Required
· AA/AS in social work, psychology, other behavioral health science field or nursing with relevant experience / training in behavioral health services.
Preferred
· BA/BS in social work, psychology, other behavioral health science field or nursing with relevant experience / training in behavioral health services.
· Equivalent combination of education and/or work experience in related field.
Years in Related Field
· 2+ years of social service, nursing or other relevant work experience.
· 1 year of supervisory or lead experience preferred.
Credentials or Certifications
Preferred
· Licensure in a mental health field
Specialized Knowledge, Skills, and Abilities and/or Competencies
· Superior interview skills necessary to obtain information about an individual’s adaptive functioning.
· Proven experience assessing the adaptive behavior of individuals with intellectual and developmental disabilities.
· Ability to effectively communicate and interact with individuals with intellectual and developmental disabilities and their caregivers.
· Excellent interpersonal, and oral and written communication skills.
· Ability to make determinations regarding an individual’s adaptive functioning and rate their adaptive functioning using software and other assessment tools.
· Strong knowledge of Microsoft Office Suite.
· Strong Knowledge or HIPAA, and Confidentiality practices.
· Experience using electronic health record systems (EHR) or other clinical documentation systems.
· Ability to enter accurate and complete data into the HER.
· Experience with Medicaid preferred.
· Experience using Vineland assessment, adaptive behavior assessment evaluation tool/software preferred.
· Verifiable good driving record and reliable transportation.
· The supervisor, will display, critical thinking abilities, strong leadership skills, wise decision-making abilities, organized, knowledgeable and excellent communicator.
· Strong knowledge of Medicaid, process and policies as well understanding of quality control management.
Leadership Competencies

Typical Job Duties and Responsibilities
· Meet in-person with eligible participants to assess qualification for the Mi Via developmentally disabled waiver program.
· Utilize Vineland assessment tool/software to evaluate domains such as communication, daily living skills, social skills, motor skills and maladaptive behaviors.
· Schedule and conduct assessments.
· Coordinate assessment sessions with eligible participants and others involved
· Complete and document Vineland assessment results in Jiva.
· Contribute to the orientation and training of other non-clinical employees
· Submit assessment results to Clinical Reviewers.
· Provide input, suggestions, and recommendations per case to Clinical Reviewers
· Participate in quality management tasks.
· Consult with colleagues and manager through regular meetings and more often as needed.
Anticipated travel and/or use of personal vehicle for business purposes.
· Frequent; To meet with eligible participants in-home or another location approved by the State of New Mexico.

Incumbents must be able to:
· Work for extended periods of time in front of a computer.
· Use a telephone, keyboard / mouse, photocopier, and other common office equipment.
· Grasp, lift, and handle documents, files, binders, and other common objects found in an office environment.
· Regularly speak and listen when interacting with others.
· May be required to travel to / from Comagine Health office to client sites and other venues; travel commitment will vary and may be outside the local area and include overnight stays.

Compensation Range: $60,000 – $75,000.
