Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).
PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:
HOME HEALTH COMFORT CARE RN
Under the direct supervision of the HH Nursing Supervisor / HH Director, the HH Comfort Care Nurse, RN is responsible for the delivery of direct participant care provided to BIENVIVIR Comfort Care participants in the clinic and at their place of residence. Responsible for the daily coordination of participant care in the following areas: participant assessments, care plans, health education, maintenance, disease prevention and management of health care with appropriate documentation in the medical record.
RESPONSIBILITIES:
1. Complete initial and routinely required assessments of individual participants to include assessment of medication compliance. Initial assessments will be completed within ten days of participants’ enrollment and upon comfort care program acceptance at initial home visit. Routine reassessments will be completed by participants scheduled interval assessment date and/ or at HH reassessment interval date.
2. Observes participant for changes in condition and reports changes appropriately to the PCP and/or designee. Obtains orders for changes in the medical plan of treatment to respond to participant’s condition, documenting all observations and interventions in an accurate and timely manner. Completes participant reassessments and updates or initiates care plan due to changes in participant condition, as expeditiously as required by their condition.
3. Completes reassessments at the request of participant/ or designated representative within 72 hours of that request.
4. Develop the care plan in conjunction with the team and provide documentation that meets the standards mandated by PACE, TDHS and nursing practice.
5. Function in the role of Comfort Care triage nurse, to include calling participants who do not come to center because they are ill or who miss scheduled clinic appointments and documenting triage findings.
6. Work closely with PCP in the daily management of participant care, checking on sick participants and assisting with procedures for Comfort Care participants as needed. Implements orders for medications and treatments prescribed by the PCP.
7. Interact and work with caregivers to coordinate participant care, and respond to participant and caregiver phone calls, to include providing education and counseling.
8. Coordinate hospital and Nursing Home admissions, discharges, and transfers for Comfort Care participants.
9. Interact and work closely with IDT, medication and treatment nurses and personal care workers to coordinate comprehensive participant care.
10. Coordinates with participant, family, caregiver and staff or contacted providers to ensure 24-hour service delivery.
11. Work closely with clinic medical assistants and field nurses to ensure all scheduled lab work and diagnostic tests are completed and results have been obtained.
12. Responsible for reviewing Lab results and provide the PCP with Lab results.
13. Audits Pharmacy med orders for Comfort Care participants daily.
14. Assures all Level II reporting guidelines/requirements are reported.
15. Documents in participants’ medical record as required by departmental policies and procedures.
16. Review and coordinate all consults and follow-up visits to include procedures and any needed medications.
17. Participate in all morning and IA meetings as necessary and daily participant meetings. To include communicating participant changes.
18. Will assist with the after-hour’s on-call rotation, and will also assist on weekends and holidays, for performing telephone triage on Comfort Care participants, addressing family and caregiver concerns, and dispatching skilled nursing on-call staff to conduct visits if needed.
19. Reports all problem areas in Comfort Care component to the supervisor, HH Director, and / or HH Administrator and conducts problem solving. 0. May assume charge duties for Home Health Nursing Supervisor, RN in the absence.
21. Attend monthly Comfort Care/ HH meetings and staff in-services.
22. Participates in QAPI activities related to the Comfort Care program.
23. When appropriate, participates in State and national organizations, meetings, seminars, workshops, and activities relating to the health profession and health care services.
24. Conducts home visits to develop and monitor the participant care plan and address the participant care needs and communicates participant status and any health changes with the IDT.
25. Performs other duties as assigned by Home Health Nursing Supervisor, RN or HH Director.
QUALIFICATIONS / REQUIREMENTS:
1. A graduate of an accredited nursing program with a license to practice in the state of Texas as a Registered Nurse.
2. One year of experience with a frail or elderly population preferred.
3. Must possess a working knowledge of management theory and practice as well as nursing theory and practice.
4. An individual with a strong knowledge base and keen understanding and appreciation of the elderly.
5. The ability to present to the community the philosophy of community-based long-term care for the elderly.
6. The skill to work collectively and communicate effectively with staff, participants, and family members.
7. Have at least one year experience as an RN obtained within the last 36 months; or ability to meet Bienvivir Senior Health Services’ conditions of employment regarding health clearance and provision of professional references.
8. Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with BIENVIVIR’S participants and their families.
9. Adaptable individual with ability to work with the interdisciplinary team approach to care for the elderly.
10. The ability to delegate responsibility as well as accept supervision in an appropriate manner.
11. Have effective telephone and interpersonal communication skills.
12. Must have an automobile, valid driver’s license, and current insurance.
13. Must be in good health and pass a physical upon employment if required.
14. Over 2 years’ experience in Home Health and/ or PACE setting preferred.
15. Bilingual in English and Spanish.
