Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).
PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

HOME CARE COORDINATOR LVN (HCC LVN)
The Home Care Coordinator LVN (HCC LVN) is responsible for overseeing a panel of participants across three centers, ensuring the coordination and quality of home care services. This role involves comprehensive assessments, interdisciplinary collaboration, and supervision of Personal Assistance Services (PAS) attendants to ensure participants receive timely, appropriate, and high-quality care in their homes.
RESPONSIBILITIES:
Participant Oversight:
1. Manage a caseload of participants from three centers.
2. Perform semi-annual assessments, including home safety evaluations and Service Plan updates.
3. Complete all documentation in the Electronic Medical Record (EMR) prior to each participant’s Care Planning date.
4. Ensure that all Care Planning information is complete for Bienvivir Participants with or without Home Care Services.
5. Communicate with the clinic triage nurse any medical emergency or change in condition in the participant's status
6. Perform Home Visits for participants
7. Semi-annual assessment visits
8. Service Plan update visits
9. Orientation Visits
10. As needed visits (status post falls, re-assessments, supervisory visits, announced visits).
Interdisciplinary Collaboration:
1. Participate in daily Interdisciplinary Team (IDT) meetings to report participant updates and concerns, regardless of participant panel assignment.
2. Participate in the Care Planning meetings for Bienvivir participants.
3. Maintain close communication with HCC peers and share relevant information brought up during IDT meetings.
4. Participate in daily huddles with the PAS team to address participant-related concerns, follow-ups, and service coordination issues.
5. Submit Service Determination Request, Incident reports, Grievances, and report incidents to the Adult Protective Services and Health and Human Services agencies as needed.
6. Participate in internal and external audits as needed.
7. Communicate closely with the IDT for any participants’ concerns, needs or issues.
Service Documentation and Coordination:
1. Review and approve Service Delivery Records (SDRs) for assigned participants.
2. Ensure SDRs are completed within 15 days of submission by Home Care Scheduling Coordinators and faxed to Medical Records upon approval.
3. Upload Service Plan Update forms into the scheduling system on the same day of completion.
4. Communicate with Home Care Scheduling Coordinators to ensure accurate and timely updates of service hours in the system.
5. Review and update the OneSource care plan list monthly.
Supervision and Staff Management:
1. Supervise a panel of PAS attendants, conducting unscheduled supervisory visits to assess performance and reinforce Service Plan compliance.
2. Provide orientation and ongoing support to PAS attendants on task execution.
3. Coordinate and complete the required documentation for 6 months and annual performance evaluations.
4. Ensure the accuracy of timesheets, manage PTO requests, administer corrective actions and performance improvement plans as necessary.
5. Collaborate with Human Resources to obtain required forms, mandatory CPR, CMS compliance employee documentation.
6. Review and approve PAS attendants’ mileage.
7. Submit the Infection Investigation form in the portal for review by the IC department.
Team & Organizational Engagement:
1. Collaborate with the HCC RN, Home Care Scheduling Coordinators, and the Home Health Administrator in the day-to-day operations of the PAS department.
2. Participate in family conferences, monthly team meetings, and PAS-specific team meetings.
3. Attend and participate in the mandatory annual Nursing Skills Fair
4. Attend mandatory monthly Bienvivir Supervisor Meetings
5. Keep excellent communication with their peers, supervisors, Schedulers, and PAS attendants.
6. Assist with covering peers' absences (PTO, sick leave).
Training & Support:
1. Collaborate with the HCC RN to train new HCC LVNs and provide ongoing support or reinforcement as needed.
2. Support Home Care Scheduling Coordinators, providing constructive feedback addressing corrections.
3. Provide active training to PAS attendants during orientation, supervisory visits, and monthly mandatory meetings.
4. Organize and actively participate in the PAS attendants’ annual Skills Fair.
Quality Improvement:
1. Participate in the Home Health Quality Improvement Work Plan as assigned by the Home Health Administrator
2. Participate in QI Committees as assigned.
On-Call Responsibilities:
1. Participate in the on-call rotation for weekends and holidays.
2. Respond to and manage PAS attendant absences or tardiness during on-call hours.
3. Ensure appropriate coverage for affected participants to maintain continuity of care.
4. Communicate with participants or their representatives for any change in schedule.
Other duties as assigned.
QUALIFICATIONS / REQUIREMENTS:
1. Licensed Vocational Nurse with current state registration.
2. Experience in Home Health Service or supervisor capacity preferred.
3. At least one year’s experience in a hospital/long-term care setting.
4. Tuberculosis test or current Chest X-Ray.
5. Current CPR certification.
