Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

 

BENEFITS for Full and Part-time employees who work 30 or more hours per week:

Bienvivir is currently accepting applications for the following position:

 

INTAKE COORDINATOR LVN

The Intake Coordinator is responsible for the recruiting and screening of prospective participants for the program. In conjunction with DHS Medicaid Eligibility Worker, the Intake Coordinator will conduct home visit and initiate the application process for all prospective participants interested in enrolling in the program. The Intake Coordinator will also serve as a program-community liaison, with local community participant referral sources (i.e. hospitals, state agencies, churches, community health care providers, local businesses, etc). The Intake Coordinator will be under the direct supervision of the Intake Supervisor.

RESPONSIBILITIES:

1. Works in conjunction with the Customer Service Representative in gathering all information needed to process an application for all potential participants.

2. In conjunction with the DHS Medicaid Eligibility worker, conducts initial screening of participants.

3. Conducts a home visit for all prospective participants who want to enroll in the program and initiates the proper paperwork for the application process.

4. Arranges participants' medical appointments and maintains open communication with the Clinic Managers regarding the appointments.

5. Arranges for participants' day of attendance to the center by coordinating arrangements with the program transportation department.

6. Arranges for multidisciplinary team assessments of newly referred participants and presents these participants cases to the multidisciplinary team for enrollment into the program.

7. Participates as an integral member of the multidisciplinary team to process the initial enrollment of recruited participants.

8. Initiates and maintains community liaison between agencies, hospitals, etc., as sources for participant referrals to the program and by scheduling presentations to such agencies as needed/requested.

9. Provides appropriate documentation for internal and external reporting purposes as required by the job.

10. Tasks and responsibilities assigned by the Intake Supervisor.

11. Assists with TILE Recertifications for annual medical necessity as needed.

12. Communicated with TMHP with regards to all pending denials and provides additional information as needed.

13. Attends medical hearings for participants who appeal TMHP denials.

14. Provides updates on potential participants' status throughout the entire application process with regards to medical and financial eligibility to the Intake Supervisor.

QUALIFICATIONS/REQUIREMENTS:

1. A graduate of an accredited institution as a Licensed Vocational Nurse.

2. A mature individual with a strong knowledge base and keen understanding and appreciation of the elderly.

3. Strong organizational skills.

4. Must have automobile, valid drivers license and insurance.
