Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:

 

MEDICAL RECORDS DATA PROCESSOR I

The Medical Records Data Processor I (DP1), under the supervision of the Medical Records (MR) Supervisor, is responsible for complying and enforcing organization and department policies and procedures, the National PACE Association (NPA) standards, Center for Medicare & Medicaid Services (CMS) and the Electronic Medical Record (EMR) guidelines. Responsible for data entry, audits, and promptly correcting discrepancies of the assigned census panel. Scans, audits, and uploads all relevant documents into the EMR systems, and the Service Utilization processes. Prepares and audits all external requested medical records in addition to the admission and monthly medical records to Nursing Home and Assisted Living (NH / AL) facilities. Serves as the back up for the Electronic Progress Note processes. Performs various clerical / secretarial functions, projects and reasonable duties as assigned.

RESPONSIBILITIES:

1. Responsible for data entry into the Electronic Medical Record (EMR). Effectively scans, uploads, and audits all relevant paper and electronic documents into the EMR Programs.

a. Audits for accuracy and completes data entry in the EMR as required (example: PACElogic demographic information, Prime Suite document upload file name, etc.).

b. Follows the EMR and scan program guidelines.

c. Scans and/or uploads paper and electronic documents and completes audit processes.

d. Prepares the purge and closed files for the assigned census panel as required; to be filed in the purge room.

e. Assists the Data Processor III (DP3) with the transfer process.

f. In the absence of a Progress Note team member, serves as the backup to the Electronic Progress Note upload processes.

g. Serves as the auditor for the Progress Note upload process.

h. Completes electronic labs process.

2. Responsible for collecting information related to Hospitals and Emergency Rooms to complete the Service Utilization Report (SUR), and to request Medical Records.

a. Collects and tracks the data for the SUR prior to submitting to the DP3 for the final data entry and report processes.

b. Verifies hospitalization and emergency room admission and discharge dates by contacting external facility staff.

c. Request, track, and complete the medical record request processes for the participant hospital stays, and emergency visits.

3. Responsible for requesting, tracking, and completing records for Visitors.

a. Completes the electronic and paper Visitors’ documents into the EMR.

b. Requests the medical records from external facilities according to the information provided in the Release of Information (ROI) form.

c. Tracks and follows up on the requests with external facilities to obtain the Visitors’ records prior to the Bienvivir Medical Doctor Assessment appointment.

4. Responsible for preparing and auditing medical records to be sent to Nursing Home and Assisted Living Facilities as required:

a. Prepares and audits the medical record packets for participant admissions and monthly medical records for nursing home/assisted living facilities and maintains copies of the packets sent as the backup record.

b. Serves as the backup for the nursing home/assisted living facilities mail-out process.

5. Serves as the DP2 backup for auditing the MPOA and Code Status documents for the assigned participants’ panel on a quarterly basis or as required:

a. Conducts the biannual audit for the medical power of attorney, advance directives, and legal guardianship documents in binder against the EMR.

b. Conductsthe biannual audit for the full code, advance directives and out of hospital, and “do not resuscitate” (DNR) code status documents in the binder against the EMR.

c. Responsible for correcting any discrepancies.

6. Meetings:

a. In the absence of the DP3, attends and obtains the daily morning meeting minutes and center information.

b. Attends the organization and department staff meetings, training, and in-services (for missed meetings, staff is responsible to meet with the MR Supervisor to obtain information).

7. Additional Duties as Assigned:

a. Prepares the monthly supply request for center operation needs.

b. Must actively participate in assigned cross-training within the MR Department.

c. Performs various projects and clerical duties as assigned.

QUALIFICATIONS / REQUIREMENTS:

1. High school diploma or GED equivalent required.

2. Certified from an accredited technical/trade school in the medical field required.

3. Two (2) years of work experience in the medical field preferred.

4. Electronic health records program experience preferred.

5. Document scanning experience preferred.
