Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:

Home Health Skilled Nursing RN - PRN

The RN is primarily responsible for the coordination of skilled home health services provided to BIENVIVIR participants at their place of residence.

RESPONSIBILITIES:
1. Coordinates participants’ skilled care in their place of residence, working closely with medical clinic staff and rehab disciplines. 
2. Accepts referrals and medical orders and documents pertinent information required for initiation of services. Assists participant/family and Primary Care Providers in arranging for skilled services in the participant’s place of residence. 
3. Implements orders for medication and treatments as prescribed by the Primary Care Provider’s Plan of Care and BSHS policy/procedure. 
4. Responsible for the completeness of Home Health records of participants, plan of treatment, progress notes, pertinent lab results, telephone orders, supervisory visits, team conferences, discharge summaries and medical updates. 
5. Assures that participants are assigned to the appropriate personnel including self, if appropriate, for evaluation, admission and on hands care. 
6. Observes participant for changes in condition and reports changes appropriately to the PCP and/or designee. Obtains orders for changes in the medical plan of treatment to respond to participant’s condition, documenting all observations and interventions in an accurate and timely way. 
7. Initiates and applies appropriate preventive, therapeutic and rehabilitative procedures, and technique. Maintains communication regarding participant’s care with the PCP, HH Nursing Supervisor, and family, as appropriate. 
8. Assists and instructs participant, family members or other participant representatives in care plan implementation and other concepts and activities necessary to promote participant’s health safety and independent living. 
9. Accurately documents observations, interventions and evaluations pertaining to participant care management and services provided in accordance with Bienvivir Senior Health Services policy and procedures on the day services are rendered. Submits documentation in a timely manner. 
10. Will be a part of the on-call rotation as well as perform visits after hours and will assist on weekends and holidays regarding answering service calls. 
11. Evaluates the clinical competence of skilled nursing personnel. 
12. Schedules visits, staffs appropriately in accordance with participant’s level of care. Assures provision of supervisory visits per required standards. 
13. Promotes utilization of all available resources for the benefit of participants, families, and team members. 
14. Plans and/or attends team case conferences with nursing staff and other BIENVIVIR disciplines as needed. 
15. Participates in staff recruitment, selection, orientation, and in-service education. 
16. Participates in the development of all BIENVIVIR objectives, standards of care policies and procedures, which affect Home Health services. 
17. Participates in the interdisciplinary team’s development, implementation, and timely revision of the individualized care plan for participants receiving HH services. 
18. Updates/maintains licensure documentation and HH manual for home health provisions assuring compliance with standards. 
19. Participates in QI activities related to skilled services. 
20. Works closely with home health contractor(s) providing services to Bienvivir participants. Assures proper documentation submitted in timely manner. Posts visits in MSUR monthly. 
21. Reports all problem areas in SN component to the Home Health Nursing Supervisor and conducts problem solving. 
22. When appropriate, participates in State and national organizations, meetings, seminars, workshops, and activities relating to the health profession and health care services.


Required Experience

QUALIFICATIONS / REQUIREMENTS: 
1. Graduate from an approved professional accredited school of nursing, licensed to practice in the State of Texas. Currently licensed (in good standing) through the State Board of Texas as a registered nurse. 
2. Meet hiring criteria as defined in Bienvivir Senior Health Services’ standards. 
3. Have at least one year experience as an RN obtained within the last 36 months; or 
4. Ability to meet Bienvivir Senior Health Services’ conditions of employment regarding health clearance and provision of professional references. 
5. Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with BIENVIVIR’s participants and their families. 
6. Must accept responsibility for maintaining clinical practice skills, learning, and adhering to Bienvivir Senior Health Services’ policies and procedures. 
7. Current CPR card. Current TB test and/or chest X-ray. 
8. Leadership ability and skills required for effective telephone and interpersonal communication. 
9. Knowledge of regulatory entities governing Bienvivir Senior Health Services H.H. policies/procedures. 
10. Bilingual in English and Spanish.
11. Must have automobile, valid driver license and current insurance. 
12. Over 1 year experience in Home Health setting.
